A Community Health Centre (CHC):

• Is a non-profit organization
• Is governed by a Board of Directors made up of community residents.
• Employs an interdisciplinary team of health professionals to provide primary health care, illness prevention and health promotion programs and services that promote wellness in the community.
• Is funded by the Ontario Ministry of Health, through regional health corporations called Local Health Integration Networks (LHINs) - not through OHIP - to provide services to residents of a geographically defined 'catchment' or services area.
• Directs most of its programs and services to 'priority populations' - groups of people who face barriers to health and barriers to accessing health care.

There are many Community Health Centres across Ontario. Some CHCs are in cities and some CHCs are in rural areas. Many CHCs access funding from other provincial, federal and municipal government funding sources. Some CHCs also receive funds from different grant programs. By accessing other types of funding, some CHCs expand their programs and services to provide supports in areas such as legal services, housing, employment, literacy, food security, prenatal nutrition and child development.
