Becoming a Member
Woolwich Community Health Centre is incorporated as a non-profit corporation and welcomes members who are:
- Interested in furthering the objectives of the corporation
- At least 18 years of age
- Residents of the health centre catchment area
Members attend the Annual General Meeting to hear and respond to reports of Board actions. Members also elect Directors to the Board. All members are eligible for nomination to the Board.
Membership in Woolwich Community Health Centre does not give you preference of treatment, care or privilege over any other health centre user.
Woolwich Community Health Centre encourages you to become a member and support our efforts in building a healthier tomorrow for us all.
If you want to become a member, please complete the membership application (click here - PDF document) and return it, with your fee, to WCHC's Administrative Secretary in St. Jacobs.
Community Feedback
We want to involve the community in the ongoing development of programs and services. We ask community members to participate in focus groups, advisory groups and to complete program evaluations to provide feedback. This information is very important in developing programs and services that meet the needs of our community.
Yearly, we ask all visitors and clients to complete a
Client Satisfaction Survey. This survey gives us specific input about our programs and services. The information is reviewed by our Management Team and guides program or service changes.

Feedback Box is kept in the front lobby of each health centre for you to make comments and suggestions.
If your concern is urgent, we encourage you to contact the Program Coordinator, Primary Health Director or the Executive Director.
